This study took 300 high risk diabetes home health care patients and provided them with remote monitoring devices.

Findings include:

The program used connected devices to integrate patient data - including blood pressure, weight, and glucose levels - directly into the care team’s database and the patient EHR. Patients could also view their data through an online portal, increasing confidence in managing their conditions at home.

Findings include: 

  • 50% reduction in readmissions for CHF patients
  • Average A1C reduction of 1.5% among diabetes patients
  • 69% of hypertensive patients improved blood pressure control through consistent monitoring
