Diabetes Care
Real time data to monitor and track
glucose levels for diabetes management
Diabetes Management & Monitoring
Diabetes management depends on daily self-care and sustained lifestyle changes that happen outside the clinic. Life365 supports self-management by giving patients visibility into their health data - helping them understand how everyday choices affect their condition.
When in-person visits occur, clinicians have data and longitudinal trends readily available, making visits more focused and productive. Automated notifications alert care teams to concerning highs or lows between visits, enabling timely guidance when needed. The result is a collaborative model that empowers patients while ensuring clinicians can intervene before issues escalate.

Diabetes Care Kit
The Life365 Diabetes Care Kit provides devices that monitor the necessary vitals to help maintain healthy glucose levels and near-real time data for care teams to provide timely interventions when needed.
The Life365 Diabetes Care Kit Includes:
-
Glucose Monitor
-
Blood Pressure Monitor
-
Weight Scale
-
Cellular Hub
"Both our organizations share a similar goal of providing people with the tools they need, extending the period of time they can live at home independently and away from acute care."
Life365 Case Studies
Using RPM to engage high-risk diabetes patients
This study took 300 high risk diabetes home health care patients and provided them with remote monitoring devices.
Findings include:
- 86% of patients improved A1C or LDL levels
- 89% of patients with A1C>10 showed improved levels
- 68% of patients with A1C b/w 8-9.9 improved levels
- 75% of patients with A1C b/w 7-7.9 improved levels
Improving ability to self-manage chronic conditions at home improves outcomes
The program used connected devices to integrate patient data - including blood pressure, weight, and glucose levels - directly into the care team’s database and the patient EHR. Patients could also view their data through an online portal, increasing confidence in managing their conditions at home.
Findings include:
- 50% reduction in readmissions for CHF patients
- Average A1C reduction of 1.5% among diabetes patients
- 69% of hypertensive patients improved blood pressure control through consistent monitoring
Proactive remote homecare program yields better patient health – leading to lower hospitalization
117 Home health care patients with diabetes were given glucometers, blood pressure monitors, pulse oximeters, and scales to measure their health trends.
Findings include:
- 91% of patients improved their blood sugar levels
- Hospital re-admission decreased in 85% of patients
