Life365 provided remote patient monitoring as part of White Plains Hospital’s innovative care model for heart failure patients. The program combined remote patient monitoring with tele-pharmacy services and community paramedicine visits, achieving a 2.6% readmission rate—well below the national average. Reducing ER Utilization for High-Risk CHF Patients
Life365 supported American Medical Response’s community paramedicine program focused on monitoring high-risk congestive heart failure (CHF) patients following hospital discharge. The program combined remote patient monitoring with in-home visits by specially trained paramedics and 24/7 virtual nurse command centers.
